WITC Transcripts

To request a copy of your WITC Transcript, please:

Download WITC Transcript Form (pdf form requires Adobe Reader to view and complete)
Key the information directly into the form and save to your computer, then attach it to an email.

-OR-
Print the form, write in your information and then mail or fax the completed form.

NOTE: Fax, e-mail, and mailing address information can be found at the bottom of the WITC Transcript Request Form.

Or if you have access to MyWITC (your personal portal), you may request a transcript using this online service.
The option to request a transcript through MyWITC is not available at this time. Please stop back.
